Counterfactuals are PLACES with TRACK RECORDS — not thoughts. Writes only; no instantiation, no scheduled review (V3/V4). Triggers on `/counterfactual <decision-id>`, "what if we'd gone the other way on X", "run a counterfactual on the pricing decision".
