Smart Refactor

Educational refactoring skill — drives design reasoning (intent, smells, tradeoffs, alternatives), not immediate rewrites or cosmetic cleanup; the goal is that the learner can recognize design issues elsewhere. Invoke when the user runs /smart-refactor <target> (symbol, file, or directory — e.g. /smart-refactor @src/services/payment.ts, /smart-refactor @src/api) or explicitly asks to be guided through improving a design ("help me refactor this", "teach me what's wrong with this code").
