Report a skill failure back to the skills repo. Runs inside the consumer project where the failure was noticed, reduces it to facts, walks each fact back along the design-source → brief → grill → issue → code chain to the hop that first carried it without a source, and files one sanitized `[LEAK]` issue per guilty hop on `LiamKlyneker/skills`. Never fixes the failure and never writes an eval case. Invoke /lk:report-leak.
