Run before meaningful releases, after major architectural changes or P0 incidents, when drift signals surface (stale TODO / hard-to-navigate codebase / avoidance behavior), or as an occasional cadence (rough monthly-to-quarterly hum). Supports full-scope (parallel sub-agents per phase group), group-scoped, dimension-scoped, --resume, and --tracker modes.
