Refuses if the eng review did not APPROVE. Logs one lightweight learning entry per attempt natively; richer capture (multi-entry retrospectives, pruning, cross-skill aggregation) and harness-policy gating are left to a wrapper — teams that want those layers should wrap this skill rather than fork it.
